The Community Pharmacy Patient Questionnaire
CFEP is an independent provider of the annual pharmacy patient survey. The
National Pharmacy Association, recommends us to any of their members who seek a
well-managed, high quality survey at a reasonable cost.
We can survey individual pharmacies, groups of pharmacies or any pharmacies
within a PCT. We can offer organisation-level reports as part of a competitive
group cost.
Please note that the prices quoted below are for a fully inclusive product -
there are no hidden extras.
The Community Pharmacy Patient Questionnaire was developed by the Department
of Health to help pharmacies to monitor quality of advice and services given
to customers. It fits onto one sheet of A4 paper (both sides)
We send you everything you need to make running the survey simple.
We send you a survey pack with questionnaires, envelopes, a poster, ballot
boxes, leaflets, clipboards and pens. Someone at the pharmacy hands the
questionnaire to a representative sample of customers (who received an NHS
service) and asks that they complete the questionnaire and put it into the
ballot box provided. We also provide self-adhesive labels with details to allow
your patients to fill in the survey on-line. You simply attach a label to a bag
containing a medicine or prescription. We also give you some questionnaires with
pre-paid CFEP envelopes, so that you are able to give a questionnaire to
patients who receive home deliveries.
Once you have given out the questionnaires, simply return the ballot box to
CFEP in the envelope provided. Our methodology guarantees patient
confidentiality and helps to ensure accurate data for you.

- 0 – 2000 scripts per month = £120
- 2001 – 4000 scripts per month = £174
- 4001 – 6000 scripts per month = £180
- 6001 – 8000 scripts per month = £198
- 8000 + scripts per month = £219
- Costs include your results information
Pharmacy results include all written comments and full colour tables and
graphs with national benchmarks derived from other pharmacies. You will also
receive a customised results poster and support material to help you act on your
results in a way that can help further your professional development and satisfy
your PCT.
